Iron Infusion Without a Referral in Ontario: How It Works

Iron Infusion Without a Referral in Ontario: How It Works

If you've been searching for information on getting an iron infusion without a referral in Ontario, you're not alone — it's one of the most common questions we hear at Ferrum Health. Many people assume that seeing a family doctor first is a mandatory step before any kind of specialist treatment. At Ferrum Health, an Oakville clinic focused exclusively on iron infusion and iron deficiency care, that isn't the case. Patients can begin their care journey directly, through a self-referral pathway led by our Nurse Practitioner team — no family doctor referral required.

This article walks through why a referral isn't always necessary, how the self-referral process actually works, and what to expect if you're considering an iron infusion in Ontario without going through a family doctor first.

Why People Assume a Referral Is Required

In Ontario, many specialist and hospital-based services do require a physician referral — it's a familiar part of how the health-care system usually works, so it's a reasonable assumption that the same rule applies to iron infusion care. But iron deficiency assessment and treatment doesn't have to follow that exact path.

Nurse Practitioners in Ontario are regulated, independently licensed health-care providers. They can assess patients directly, order relevant bloodwork, and — when appropriate — refer a patient onward to an Internist-led clinic for further diagnosis and treatment. That means the entry point into care doesn't have to start with a family doctor at all.

How Self-Referral Works at Ferrum Health

Getting started doesn't mean tracking down a referral form or waiting weeks for a family doctor's appointment. Our self-referral pathway is designed to be simple and clear:
  1. Complete our online intake form. Share your symptoms and relevant health history through a short online form.

  2.  Nurse Practitioner Review: Your journey begins with a free consultation with one of our licensed Nurse Practitioners. After reviewing your medical history and symptoms, they will determine the most appropriate next step. This may include referral to our Internist-led Iron Deficiency Clinic or, when clinically appropriate, ordering blood work and prescribing intravenous iron treatment directly.

  3. We contact you to book. Once your care pathway is confirmed, our team reaches out to schedule your appointment.

There's no waiting room to sit in, no paperwork to chase down, and no requirement that you already have a family doctor. This is what makes an iron infusion without a referral in Ontario genuinely accessible for people who might otherwise put off getting checked.

Why This Matters More Than Ever

As of September 9, 2024, Ontario's major community laboratories updated the clinical decision limits used to flag low ferritin — the blood marker used to screen for iron deficiency — raising the threshold from the previous range of under 10–15 µg/L to under 30 µg/L for adults.¹ In practical terms, this means many Ontarians who were previously told their iron levels were "normal" may now meet the clinical threshold associated with iron deficiency.²

That's a meaningful shift — but it also means more people may benefit from getting assessed than realize it, and many simply don't know how to act on it. A self-referral pathway removes one of the biggest practical barriers: not having quick access to a family doctor to start the conversation.

If You Already Have a Family Doctor or Specialist

Self-referral is one option, not the only one. If you already have an established relationship with a family physician or specialist, Ferrum Health also accepts direct provider referrals and infusion orders by fax, including for physicians who want to stay closely involved in your care. Our Providers page outlines these referral pathways for health-care providers.

What to Expect at Your Intake

  • Recent bloodwork helps, but isn't required to start. If you have recent ferritin, CBC, or iron studies, bring them along — if not, our team can help you arrange testing.

  • Be ready to describe your symptoms. Fatigue, brain fog, hair loss, and shortness of breath are among the most common reasons people reach out (more on this in our related article below).

  • A Nurse Practitioner determines next steps. This may include a referral to our Internist-led Iron Deficiency Clinic for iron infusion if clinically appropriate.

  • Costs are explained upfront. All medications are coordinated through our associated pharmacy team, and you'll be informed of coverage details and any out-of-pocket costs before your appointment — no surprises.

  • Free Nurse Practitioner Assessment. If medically appropriate, your Internist consultation is fully covered by OHIP, providing timely assessment and treatment for iron deficiency and intravenous (IV) iron infusion.
